Help children and adults
overcome attention disorders
Attention-deficit/hyperactivity disorder can be difficult to diagnose,
yet proper intervention and treatment can have a profound impact on
the lives of patients with attention disorders.
During an Assembly case study session on children and adults with
attention disorders Sept. 16 in Orlando, Fla., two experts in the
field presented overwhelming evidence that even patients with severe
AD/HD can be helped.
"Diagnosis at an early age is critical, because the earlier you can
intervene the better chance the patient has to develop skills to cope
and find success in life," said Clare Jones, Ph.D., a nationally known
expert on AD/HD and the author of four books on the subject. "It's
important for the patient and family to develop a positive attitude.
The message is, 'You can help this unique child survive and thrive.'"
Jones and James Nahlik, M.D., associate clinical professor of
community and family medicine at St. Louis University Medical School,
discussed six case studies of patients ranging in age from 5 years to
adult. AD/HD is a lifespan disorder, Dr. Nahlik said.
"The symptoms may change over a patient's life, but children do not
outgrow AD/HD," he said. "You can diagnose AD/HD at any time during a
patient's life."
Jones noted that mild attention disorders are the most difficult to
diagnose, because patients often develop their own coping skills.
"Many patients with mild AD/HD never realize they have the disorder
because they have been able to deal with it effectively on their own,"
she said. "Thus the patients you are most likely to see in your
practice will have moderate to severe AD/HD and definitely need your
help."
Behavior modification, psychotherapy, family therapy and support
groups can be indicated for AD/HD children and their families, Nahlik
said. Family therapy is important, he noted, because some 70 percent
of children with attention disorders have one or both parents with the
same disorder.
He went on to discuss some of the commonly prescribed medications for
AD/HD, including psycho stimulants such as the commonly prescribed
Ritalin® and Cylert®.
"Psycho stimulants are beneficial in 70 to 80 percent of patients with
AD/HD, but parents often ask us, 'Why stimulate the hyperactive
child?'" Nahlik said. "The explanation is that these medications
specifically stimulate the part of the brain involved with attention."
Although medications are beneficial, both speakers recommended a
"skills, not pills" approach to treatment.
"Medication can be helpful as part of the process of helping a patient
learn to focus and stay on task," Jones said. "Just writing a
prescription isn't enough -- you have to empower your patients to help
themselves."

Health concerns show no sign of slowing the wireless expansion. So far
only a few, faint warnings have been raised, mainly by people who are
so sensitized to the electromagnetic radiation emitted by mobiles,
their masts and Wi-Fi that they become ill in its presence. The World
Health Organization estimates that up to three out of every hundred
people are "electro sensitive" to some extent. But scientists and
doctors - and some European governments - are adding their voices to
the alarm as it becomes clear that the almost universal use of mobile
phones may be storing up medical catastrophe for the future.
A recent authoritative
Finnish study has found that people who have used mobiles for more
than ten years are 40 per cent more likely to get a brain tumor on the
same side of the head as they hold their handset; Swedish research
suggests that the risk is almost four times as great. And further
research from Sweden claims that the radiation kills off brain cells,
which could lead to today's younger generation going senile in their
forties and fifties.
There has been less
concern about masts and relay antennas, as they emit very much less
radiation than mobile phones. But people living - or attending schools
- near them are consistently exposed and studies reveal a worrying
incidence of symptoms such as headaches, fatigue, nausea, dizziness
and memory problems. There is also some suggestion that there may be
an increase in cancers and heart disease.

Microsoft runs the I.R.S.
If Microsoft Ran The IRS
"Government should be run like a business." We've all heard that
chestnut. Here is how the Internal Revenue Service (nobody's favorite
government agency) would be like, if only it were run like Microsoft
Corp. (a successful private enterprise).
-- The IRS, as always, announces new tax forms will be mailed the week
before the new year. However it will follow Microsoft's example and
actually ship them the following May.
-- Responding to pressure from some large corporations and a users'
group, some early copies of the tax forms will actually be released in
March. The recipients must sign non-disclosure agreements.
-- In June, the forms will be recalled because the IRS loses a suit for
appropriating some other country's intellectual property.
-- When you move, the IRS will continue to send mail to your previous
address forevermore, just like Microsoft sends its product upgrade
notices.
-- When you upgrade from form 1040 EZ to 1040 A, and then to 1040, you
will pay an upgrade fee each time. Also you need to send in a new
registration card and get a new Social Security Number. In order to
upgrade, you have to submit the original first page of your previous
year's form.
-- Like Microsoft, when you file a late or amended tax return the IRS
will reject it on the grounds that the the prior year is no longer
supported.
-- The IRS telephone help will remain similar to Microsoft's, staffed by
ill-trained, high-turnover personnel who sometimes give a correct
answer, but the IRS will have to discontinue using a toll-free phone
number.
-- After struggling with reams of dense documentation of complex options
and rules, you discover that you will need publication 3297, with a
ten-word-long title, in order to answer (you hope) a single obscure
question. The IRS, like Microsoft, will charge a minimum of $40 for that
publication.
-- The IRS, like Microsoft, will continue to issue immense volumes of
bug fixes, interpretations, and clarifications. However the tax-rule
updates should be neither easily searchable nor well-indexed.
-- Instead of three-ring binders containing complete sets of tax code
bugs and interpretations, IRS rulings will be promulgated in a haphazard
fashion by individual taxpayers via BBS, Usenet, and Compuserve. A for-
profit publishing subsidiary would also be nice.
-- The new all-powerful (and eccentric) Commissioner of Internal Revenue
will jet around the country giving speeches and granting numerous
interviews, but only to sycophantic reporters. Changes to the tax code
will be at the whim of the Commissioner and largely kept secret until
they are published.
